The Aged Care Liaison Service has seen significant improvements to the health care experience for aged care residents by providing care in their clients' own environment, thus avoiding the need for unnecessary time spent in Emergency Departments, preventing admission to hospital, and reducing the length of stay and risk of functional decline if admission is required.
