Cognitive Dementia & Memory Services (CDAMS)

A CDAMS clinic is a specialist service providing early diagnosis, advice, support and referral for people with cognitive difficulties causing confusion, memory loss or thinking problems.

SubAcute Ambulatory Care Services (SACS) is the overarching name for services provided by Community Based Rehabilitation
and the Specialist Clinics to non-admitted patients. All the
services run through SACS offer a co-ordinated multi disciplinary approach to patient care.

 
Community Based Rehabilitation

Community Based Rehabilitation (CBR) works with clients over
the age of 16 years who are disabled, frail, chronically ill or
recovering from traumatic injury. CBR staff, together with the
client, set goals that will increase their independence develop
a care plan to meet these goals and assist with re-integration
into the community. Services can be provided at either the
Sunshine Centre, the Williamstown Centre or in the client's
home, depending on the goals and care plan.

Continence Clinics

The Continence Clinic offers a clinical service specialising in incontinence and other bladder and/or bowel function difficulties.
It provides assessment, diagnosis, management, education
and support to improve continence for clients.
