Information for carers/care facilities

Patients that are attending Western Health Outpatient Services from 24-hour care facilities such as nursing homes are to be escorted to the Outpatients Department. The escort can be a family member or a professional. If an escort is not provided the patient will be returned to your facility without being medically reviewed. If there are any concerns please feel free to contact the relevant outpatients department.

In outpatients we make every attempt to expedite patients from care facilities through the clinics but sometimes there can be delays. A nursing transfer letter will be given to the patient to be filled in by the facility for the next visit. This is to ensure continuity of care. In the department our staff will assist with any questions or concerns you may have.
