Western HARP Chronic Disease Management Mission
To create an integrated, effective and sustainable chronic disease and complex needs program, which provides the right care in the right setting, to reduce avoidable hospital admissions and contribute to better health outcomes for the Western Population
The Western HARP CDM Vision
Clients will be able to access services from anywhere in the catchment and that those services will be:
- Provided in a way that ensures equitable access for all clients
- Integrated with usual services across catchment
- Responsive to client need
- Evidence based
The shared governance model will continue to ensure that resources are shared equitably across region and based on client need.
Shared information management systems will allow clients and associated resource usage to be tracked throughout the region.
