The Division provides a range of evidence based, client centred, specialist services in the Emergency Department, acute inpatient, subacute and community.
It undertakes comprehensive assessment and care planning, to optimise an individual's function and wellbeing as well as plan and deliver the effective transition of care from the physical, social and psychological perspectives. It also coordinates the care and transition of patients throughout the emergency, inpatient and community based settings.
