What to bring to your outpatient appointment?

  • The appointment letter
  • Medicare card
  • Traffic Accident Commission (TAC) number
  • Blood test results
  • Up to date list of medications
  • X-ray/scan results
  • WorkCover number
  • Pensions/concession cards
  • Department of Veteran Affairs card

  

Diagnostics/Medications

It is essential that you bring all recent x-rays, scans and blood tests to your appointment. Please ensure that you have an up to date list of all your medications for the doctor. Your local doctor will be able to give this to you.


Missed Appointments

If you fail to attend an appointment without notifying the hospital you may be discharged from the clinic and therefore require a new referral from your General Practitioner. Every effort will be made to contact you either by telephone or post, and your local doctor will also be notified. We have a high demand for all our clinics so it is really important that we are notified if you cannot attend or if your details have changed.


Change of Contact Details

If you move home and/or change your contact telephone numbers, please advise the Western Health Outpatients Department as soon as possible.
