The entry criteria for Western HARP CDM is those people who
have frequent presentations to ED and/ or at risk of frequent presentations, with defined chronic diseases & complex needs.
These are people who are most likely to benefit from integrated
care and have the potential to reduce avoidable hospital use.
This includes:
