The Community and Ambulatory Care department:provides;

• Assistance to patients to transition, recuperate and/or rehabilitate post hospital episode. 
• Direction to patient care in clinics, in community centres and in patients' homes
• Co-ordinated, multi-disciplinary care including brokerage services
• Lead electronic GP messaging system
• Establish care pathways between GPs and acute sector
• Build on strong relationships with GPs and Divisions of GPs through targeted projects
