The Community and Ambulatory Care department:provides;
- Assistance to patients to transition, recuperate and/or rehabilitate post hospital episode.
- Direction to patient care in clinics, in community centres and in patients' homes
- Co-ordinated, multi-disciplinary care including brokerage services
- Lead electronic GP messaging system
- Establish care pathways between GPs and acute sector
- Build on strong relationships with GPs and Divisions of GPs through targeted projects
